Change of name on marriage

In the event of a change of name due to marriage, employees are welcome to first contact the responsible person in the HR department by e-mail. To report a name change, the original certificates must be submitted once. The CHANGE OF NAME NOTIFICATION form below does not need to be completed if the original certificate can be presented.

Birth of a child

In the event of the birth of a child, employees are welcome to first contact the responsible person in the HR department by e-mail. To report a name change, the original certificates must be submitted once. The CHANGE NOTIFICATION form below does not need to be completed if the original certificate can be presented.

Change of bank details

In the event of a change of bank details, employees are welcome to contact the responsible person in the HR department by e-mail first. To report a change in bank details, you must submit an original signed document (e.g. either a CHANGE NOTICE or page 1 of the SEPA account details).

Change of address

You do not need to fill out a form to notify us of a change of address. Please simply write an e-mail with the change to the responsible person in the HR department. The information will then be forwarded to the NLBV.
